USA, Indiana: After a blown transformer caused an underground fire along Massachusetts Avenue in Indianapolis on Monday night, Indianapolis Power and Light (IPL) engineers said there were no issues with the transformer when they last inspected it in October 2014.

The 35-year-old transformer exploded in front of a Starbucks Coffee shop causing a large boom and a fireball eruption from the sidewalk, reports Indy Star.

The problem apparently occurred in the primary termination chamber where cables from under the street enter the transformer.

IPL plans to remove the transformer for inspection and replace it with a new one.
